유소아난청 평가용 특수청각검사 : BAEP, BERA 그리고 ABR

안녕하세요~

저는 신생아 중환자실에서 일하고 있는 간호사입니다.

병원에서 일하다 보면 미숙아들 신생아 청력검사를 필수로 하게 되는데요.

일하다 보니 BERA 검사와 BAEP 검사 두가지가 나뉘어서 있더라구요.

차이점에대해서 병동 선생님들과 함께 공부하려고 공부하다 보니,

ABR이라는 공식명칭으로 불리고 있다는 걸 알게되긴 했는데요,

둘의 차이점에 대해 발표하려다 보니..ㅠ

차이점에 대해서는 자료를 찾기가 어려워서요.ㅠ

저의 세부 전공이 특수청각검사 분야가 아니어서 그 동안 배운 내용과 최신 자료를 정리하여 답변을 드리겠습니다.

 

장수진 간호사님께서 질문하신 내용은 BERA 검사와 BAEP 검사의 차이점이 무엇인가? 로 요약이 됩니다.

 

자료에 의하면 1979년 Auditory Response from the Brainstem을 주제로 열린 세미나에서 BERA(Brainstem evoked response audiometry), BAEP(Brainstem auditory evoked potential), BSER(Brainstem Evoked Response)등으로 사용되던 뇌간유발반응 용어를 ABR(Auditory Brainstem Response, 청성뇌간유발반응)로 공식 명명한 뒤 지금까지 ABR로 불리어오고 있습니다. 따라서 BERA(Brainstem evoked response audiometry)와 BAEP(Brainstem auditory evoked potential)는 검사방법, 결과 해석 방법이 유사한 검사 종류로 보시면 될 것 같습니다.

 

그러나 실제 BAEP, BERA는 검사방법과 해석은 같지만 사용하는 목적에 따라 다소 다르게 불리어 오고 있는 것으로 조사되었습니다.

 

BAEP(Brainstem auditory evoked potential)

청각의 손상은 뇌막염, 고빌리루빈 혈증, 주산기질 등과 같은 중추 신경계 질환에 의할 수 있습니다. 즉 신경계 손상으로 청력 손실이 있을시 신경계 손상을 판단할 때 사용하는 뇌간유발반응 검사를 BAEP 검사라고 합니다.

 

BERA(Brainstem evoked response audiometry)

신경계 검사가 아닌 단순히 청력검사를 목적으로 사용 되면 BERA 검사라고 합니다.

 

결론적으로 신경계검사의 일종인 ‘BAEP’안에‘BERA’검사가 포함되어 있다고 보시면 이해하시기 쉬울 것입니다.

Brainstem Auditory evoked potentials (BAEPs) are very small electrical voltage potentials which are recorded in response to an auditory stimulus from electrodes placed on the scalp. They reflect neuronal activity in the auditory nerve, cochlear nucleus, superior olive and inferior colliculus of the brainstem. They typically have a response latency of no more than six milliseconds with an amplitude of approximately one millivolt. Due to their small amplitude 500 or more repetitions of the auditory stimulus are required in order to average out the random background electrical activity. Although it is possible to obtain a BAEP to a pure tone stimulus in the hearing range a more effective auditory stimulus contains a range of frequencies in the form of a short sharp click.

Long and Allen reported the abnormal BAEPs in an alcoholic woman who recovered from Ondine's curse. These investigators hypothesized that their patient's brainstem was poisoned, but not destroyed, by her chronic alcoholism.

BERA (Brainstem auditory evoked response audiometry) is an objective way of eliciting brain stem potentials in response to audiological click stimuli. These waves are recorded by electrodes placed over the scalp.Thisinvestigation was first described by Jewett and Williston in 1971.

Even though BERA provides information regarding auditory function and sensitivity, it is not a substitute for other methods of audiological evaluation. It should be always viewed in conjunction with other audiological investigations.
