유소아 난청아동의 경우 주관적인 검사가 어려워 잠을 재워서 하는 특수청각검사인 ABR 검사를 수행합니다. ABR 검사는 객관적 청각검사로도 일컬어지는데 이는 대부분의 난청센터나 보청기전문센터에서 행해지는 검사 결과치와는 다소 차이가 있습니다.

국내 특수청각검사 ABR 검사를 수행하시는 전문가들에 의하면 순음청력검시 결과치에 비해 10~15dB 정도 높다고 이야기를 합니다. 즉, ABR 검사에서 60dB는 순음청력검사치로 환산하면 대략적으로 45~50dB가 된다고 합니다. 


해외문헌을 찾던 중에 ABR 검사 결과치와 보청기 적합(fitting)을 위한 순음청력역치의 주파수별 상관관계를 도식화한 그래프가 있어 포스팅합니다.

그래프를 살펴보면 저주파영역이 고주파영역에 비해 두 값의 차이가 큼을 알 수 있습니다.
즉, 500Hz에서는 20dB, 1000Hz에서는 15dB, 2000Hz에서는 10dB, 4000Hz에서는 5dB의 값이 차이가 납니다.  

 

How are the corrections applied to the ABR values?

The frequency-specific corrections are subtracted from the ABR values. Since previous research has shown that ABR threshold estimates (nHL) are typically higher than behavioral thresholds (HL), the correction value is subtracted to provide a better estimate of behavioral thresholds (eHL) for the purposes of hearing instrument fitting. The schematic in Figure 1 describes this calculation, which is also represented on the accompanying audiogram.

The correction values are frequency-specific and are impacted by how the ABR system is calibrated, as well as the stimulus and collection parameters used. For some prescriptive algorithms (i.e. DSL m[i/o] v5), the clinician can 1) enter data in nHL and a default correction will be applied, 2) enter data in nHL and apply clinic-specific corrections, or 3) enter the data in eHL. The third method assumes that a correction has already been applied and the algorithm will not apply one.
